Class Placement Form

This form is designed to provide information to Marist Regional College, to assist in the decision-making process when considering specific requests of parents/guardians to have their child placed with a friend to assist with the transition into the new school year (requests need to be supported with documentation from a medical professional or information from the previous year). This form can also be used in the event there is a student you feel will have a negative impact on your child's learning or wellbeing if they are placed in a class with that student (there needs to be previous documented evidence at the College and/or from a previous school for these requests to be considered). It is acknowledged by the College that friendships are important, however, there are many complex factors that need to be considered when placing students into class groups. The College will consider all requests made but cannot guarantee that they will be able to be accommodated. Collection and storing of information and documents is in accordance with the Catholic Education Tasmania Privacy Policy which can be accessed at https://catholic.tas.edu.au/policies

If you have provided responses in questions 6 - 11 and you have indicated in your reasoning the request relates to a medically diagnosed condition, please upload supporting documentation as provided by the medical practitioner. Should you wish to provide the supporting documentation via email, please send it to Mrs Sherry Rand at srand@mrc.tas.edu.au. Documentation provided will be held in the strictest of confidence. As the information you provide is used in consideration of class allocation, the information you share will be distributed to those members of staff deemed appropriate.
